My Story

The Community Connectors will work within the compassionate community team at Keech Hospice Care and alongside individuals in Luton from South Asian community, including Pakistan and Bangladesh communities.

They will undertake collaborative working amongst local partners recognising the value of community groups. They will work collaboratively with partners to co-create compassionate communities through an “asset based” community development model

The Community Connector will work with organisations, community groups and community members to promote conversations about death, dying, loss and bereavement. This will include planning for ill health. They will provide access to and knowledge about palliative care and hospice services.

Identify and build relationships with community groups, faith organisations, voluntary groups and business organisations within the south Asian community in Luton.

To work with organisations and groups to promote events and activities that promote awareness around death, dying and loss.

Organise local meetings to introduce communities to our work and aspirations.

Capture local stories of community activity and share these to provide inspiration and demonstrate what is possible.

Provide learning and confidence building to local people on death, dying and bereavement loss.

To work with individuals to support conversations about planning ahead, making a will, planning a funeral and access to bereavement support.

Help to promote and encourage engagement with partners across Luton.

Meeting with people in a variety of settings, including community centres, people’s homes or other appropriate settings.

To help raise awareness of palliative and end of life care services. including Keech Hospice care and were appropriate, support referrals to these services.

Help develop local volunteer networks of support in the community.

Support and assist with the monitoring and evaluation of the activity as well as collecting broader qualitive and outcome data.
